Case Manager (Home for Good)


Help Louisville turn housing goals into real homes.

Wellspring is partnering with Louisville Metro’s Home for Good initiative to help move individuals experiencing chronic homelessness into permanent supportive housing. In this role, you will be part of a community-wide effort working toward housing 250 people by 2027 while providing the case management, connection, and ongoing support that helps people not only find housing, but keep it.  Join us and see the direct impact of your work in the lives of people across our community!


Qualifications:
  • Experience working directly with adults in a social service, mental health service, substance abuse service, and/or homeless service setting is preferred.
Education:
  • A bachelor’s degree in a Human Services field such as Psychology, Sociology, Social Work, Family Studies, Human Services, or Counseling is preferred.
Salary:
  • $45,000 - $52,000 annually
Shift/Schedule: Full-time, Monday - Friday

Conditions of Employment:
  • This position requires clearances of state and federal background checks, multiple abuse/sex offender registries, and clearance of exclusion lists prior to employment.
  • Must have access to a personal, registered, and insured vehicle to transport clients (when company vehicles aren’t available) and to travel between client locations and related appointments in the greater Louisville area.
  • Must possess a valid, active driver's license in good standing.
Other Requirements:
  • Home for Good (HFG) Case Managers must be prepared and comfortable working outside and in settings where our clients reside. 
  • Must demonstrate enthusiasm for working with a disadvantaged population: individuals who are homeless, disabled by mental illness and often experiencing co-occurring substance abuse and poverty.
  • Knowledge of the theories of human behavior and the principles and techniques associated with the provision of recovery-based case management. 
  • Strong ability to establish and maintain an effective working relationship with adults with a serious mental illness coming from homelessness, colleagues and agency/community providers/Metro/Home for Good committees. 
  • Ability to communicate effectively and respectfully, both orally and in writing.
  • Ability to operationalize a strengths-based, person-centered approach to case management.
  • Consistently demonstrate a strong respect for the dignity of all individuals. 
  • Working knowledge of community resources and ability to facilitate coordination activities.
