Wellspring's Crisis Stabilization Program:
The goal of Wellspring’s Crisis Stabilization Program is to assist adults who are experiencing an acute psychiatric episode. With our 24/7 care, we help people avoid hospitalization, achieve stabilization, and recover from acute mental health symptoms such as severe depression, mania, or psychosis. Clients receive intensive treatment, close monitoring with high levels of staff support, and medications prescribed by a psychiatrist, if necessary, in a homelike setting. The 16-bed program has two locations – the Samuel B. Todd Center and the David J. Block Center. Both are licensed, accredited crisis stabilization offering the only services of their kind in the Louisville Metro region. 

 
Crisis Stabilization Unit (CSU) Targeted Case Manager (TCM) Qualifications and Requirements: 
  • Valid KY driver's license and a personal vehicle are required as the CSU TCM will be responsible for traveling between work sites and transporting clients to appointments.
  • KY TCM Training
  • BA or BS in field recognized by DBHDID for TCM: Psychology, Sociology, Social work, Family studies, Human services, Counseling, Nursing, Behavioral analysis, Public health, Special education, Gerontology, Recreational therapy, Education, Occupational therapy, Physical therapy, Speech-language pathology, Rehabilitation counseling, or Faith-based education OR Master's Degree in related field.
  • Must have a minimum of year of full-time experience working directly with adults in a human service setting after completing the educational requirements. A Master’s degree in the above disciplines substitutes for one year of experience.  
  • Knowledge of the theories of human behavior and the principles and techniques associated with the provision of recovery-based case management for adults with SMI and co-occurring disorders. 
  • Ability to operationalize a strengths-based, person-centered approach to co-occurring case management. 

CSU TCM Duties and Responsibilities:
  • Completes a comprehensive needs assessment to determine needs and goals for the client to obtain and maintain stable housing and maximize self-sufficiency and self-determination.
  • Completes an individualized person-centered recovery plan focusing on both SMI and substance use. 
  • Provides linkage, referrals, and ongoing coordination and communication with community resources and support services including physical health, behavioral health, dual diagnosis recovery services, employment services, educational programs, and mainstream benefits (SSI, SSDI, SNAP).    
  • Facilitates housing applications through LMHA, the Common Assessment Team and appropriate supportive housing programs.
  • Assists clients in completing housing applications and housing search, including obtaining necessary documents (e.g., KY ID, birth certificate, etc.).
  • Collaborates with landlords and other community partners, advocating for client needs and tenant rights.  
  • Provides crisis prevention / intervention services and appropriate referrals.
  • Provides Life Skills support, as needed, around illness management, medication management, nutrition, household management, conflict management, budgeting and shopping for food / furnishings / basic needs.
  • Supervise clients' self-administered medications (SSAM), assists clients with their engagement in physical and mental health and addiction treatment and ensuring uninterrupted access to all medications as applicable.
  • Meets with clients as frequently as necessary for them to maintain housing and address the goals identified in their person-centered plan. This could range from daily during periods of crisis to weekly during periods of stability. Weekly contact is the general expectation.

Salary / Other Benefits: 
  • $40,000 -  $50,000
  • Mileage reimbursement for work related travel.
