Wellspring Program Manager FACT Summary:
Wellspring is seeking a full-time Program Manager for FACT who oversees the safety, support and wellbeing of program clients and manages and supervises a multidisciplinary team. The role leads daily care coordination, ensures evidence-based Integrated Dual Diagnosis Treatment and Housing First services, and oversees outreach, assessments, treatment documentation, referrals, advocacy and staff supervision. The Program Manager collaborates with community partners to coordinate referrals, supportive services, outreach and required program data. Wellspring Program Manager FACT Summary:
Wellspring is seeking a full-time Program Manager for FACT who oversees the safety, support and wellbeing of program clients and manages and supervises a multidisciplinary team. The role leads daily care coordination, ensures evidence-based Integrated Dual Diagnosis Treatment and Housing First services, and oversees outreach, assessments, treatment documentation, referrals, advocacy and staff supervision. The Program Manager collaborates with community partners to coordinate referrals, supportive services, outreach and required program data. The following areas are key responsibilities include but are not limited to:
- Along with the clinical team, provides assessment, individual therapy, treatment planning, and ongoing support to adults with serious mental illness and co-occurring disorders using evidence-based practices.
- Leads a multidisciplinary team, including staff supervision, coaching, training, performance evaluations, recruitment, hiring, and onboarding.
- Oversees client treatment and service coordination, working closely with psychiatric and medical providers, guardians, community partners, and other members of the treatment team.
- Supports clients with medication management, referrals, advocacy, and connection to community-based services.
- Provides Housing First services and community outreach to individuals experiencing homelessness and coordinates services with community partners.
- Ensures timely and accurate clinical documentation in accordance with agency standards, Medicaid requirements, and other applicable funding and regulatory requirements.
- Oversees day-to-day program operations, including managed care preauthorizations, program reporting, service coordination, and collaboration with administrative departments.
- Maintains relationships with Louisville Metro Housing Authority and other community partners to coordinate referrals, supportive services, data collection, and funding requirements.
- Manages and monitors the program budget.
- Works collaboratively with leadership to support effective program operations and quality client services
- Minimum of two years of progressively responsible experience providing behavioral health services to adults with serious mental illness (SMI), including experience working with individuals with co-occurring mental health and substance use disorders.
- Demonstrated supervisory, program, or team leadership is required.
- Experience with community-based behavioral health services, housing or homelessness services, interdisciplinary treatment teams, Medicaid documentation, and/or managed care authorization processes is strongly preferred.
- Certified Social Worker (CSW) is required; Licensed Clinical Social Worker (LCSW) preferred.
